New Patient Registration

The Surgery has an application system for anyone wishing to join the Practice.

Application forms are available from Reception or can be downloaded here. We ask that forms are completed and signed, then returned to the Surgery in person by the applicant, with a valid form of ID. Patients accepted on to the Practice list will be invited to attend for a ‘New Patient Check’. This is a routine examination, where we will check the patient’s height, weight, blood pressure and urine, along with taking a brief past medical history.

In order to register as a new patient with the Practice you must live within the  Core Practice Area (Green Zone) that has remained the same for many years. The outer boundary area (Purple Zone) is a previously added area to accommodate patients who have moved out of the Green Zone into that area and still wish to stay registered with the Practice.
